Once you have met with a Formal Dimensions consultant, you will be given a registration code and you can use this form to list everyone's name, phone, and email address so we can contact them and deliver their individual instructions. Once a member of your party has been fitted, you will receive an email from our staff notifying you. Two weeks prior to your wedding date, if some members have not yet been fitted, you will receive an email with those individuals' names and we ask that you contact them to make sure they get in as soon as possible.
